Hello, This download contains the Assessment Forms that you are required to fill out to be placed on the Call List. Also included are the Required Tools List and a copy of the Call-In Procedure. Please fill out the self-assessment provided, according to your area of specialty, and attach a résumé. If you wish to be placed in both categories, you must send a separate résumé to accompany the self-assessment for each category. Please mail your completed self-assessment forms to: P.O. Box 22562 300 Coxwell Avenue Toronto ON M4L 2A0 When we receive your information, you will be contacted to confirm your addition to the Call List.
